Accident, Incident and Near Miss Reporting and Your Responsibilities

Should an Accident, Incident or Near Miss occur, it needs to be reported ASAP. This can be done with your Manager or the Compliance Team who will advise what to do next.
Please include as much information as possible, including photos from all angles if possible, witness's names and contact details.
If required you will be contacted for further investigation.
If you are involved in an accident, incident or near miss whilst on a customer’s site, you MUST inform your despatcher AND report the incident to the Client BEFORE you leave the site.
REMEMBER
If you’re having trouble completing the report ask for assistance from the Compliance Team.

Serious Harm

Ensure the area is safe.
Attend to injured person.
Do not interfere with the accident scene unless to save life.
If possible take photos.
Contact your manager or the Compliance Team.
Accident must remain frozen until WorkSafe NZ has released the work area.
